It's a common issues that "maybe" may be related to internal timing issues. The first world was like this for me as well and MY best bet is that as soon as this issue begins to present itself... jump off whatever you're climbing immediately! Doing so prevents clipping through the environment and warping back to the entrance.
For me, the issue is greatly reduced when using GLideN64 alone. It's no guarantee, but it "seems" to have helped me. As I said at the top, it may be related to timing issues. These issues can apparently be resolved using GLideN64 and specific RSP plugins, but anytime I've tried the timing is always off. The dead giveaway being that the intro rap falls out of sync. Using the right plugin combo and settings, this won't happen and may solve the emulation glitch.
Edit: Noticed you say you use an old XP Dell machine. Unless you upgraded it to it's furthest my above suggestion may not work well if at all.
